Request An Appointment

Payments must be made prior to appointment. In the case of a cancellation or reschedule, this must be done 24 hours in advance or you will be charged $50.00 fee. If appointment is missed due to an emergency, documentation must be provided. Paperwork, unscheduled calls, and concierge visits are handled on a case to case basis.
